By using this website or app (as applicable) (each referred to herein as the ‘platform’), you agree to be bound by the following terms and all applicable laws and regulations. The information contained in this platform is published in accordance with and subject to all applicable laws, including Regulation 28 to the Pharmacists Regulations (Preparations), 5746- 1986 (the "Pharmacists Regulations") and the Ministry of Health's Procedure 134 for Raising Disease Awareness ("Procedure 134") and Procedure 137 regarding Rules for Improving the Use and Adherence with Prescribed Medicinal Treatment Using Non-Commercial Information ("Procedure 137").

Any information or content included and/or published on this platform is not constant and it may be subject to specific legal provisions. Subject to such applicable legal provisions, and specifically Procedure 137, Lilly shall be entitled to update and/or remove and/or amend any information or content included and/or published on this platform, in accordance with its exclusive reasonable discretion.

Target Audiences

Lilly operates various platforms, including websites intended for public awareness purposes ("Public Awareness Websites"), websites and an app designated for licensed health care professionals e.g., physicians, pharmacists and nurses (together, "HCPs") and/or websites that include designated areas for HCPs (together, "HCPs Platforms"), and websites intended for patients prescribed with a Lilly prescription drug ("Patients Websites") (Public Awareness Websites, HCPs Platforms and Patients Websites shall be together referred to as "Lilly Platforms").

Lilly also operates a landing page website, from which, subject to applicable law, users can reach and access one of Lilly Websites.

Public Awareness Websites

Public Awareness Websites are intended and accessible to the public for increasing public awareness regarding a certain disease (e.g., Obesity, Diabetes, Alzheimer) and to inform the public about such disease, including, inter alia, symptoms, risk factors, possible treatments, complications prevention, etc.

To the extent this platform is a Public Awareness Website, and you are not using it as a patient prescribed with a Lilly prescription drug or an HCP, the information contained on the platform is published in accordance with and subject to all applicable laws, including Regulation 28 to the Pharmacists Regulations and the Israeli Ministry of Health's Procedures 134 and Procedure 137 . As aforesaid, Lilly provides such information for public awareness only, and it shallnot be interpreted or considered as provided for any other purpose. For the removal of any doubt, the content of such platform is in no way intended to be a substitute for professional medical advice and should not be interpreted as treatment recommendations. Neither the content nor any other service offered by or through this platform as a Public Awareness Website is

HCPs Platforms

Given Regulation 28 to the Pharmacists Regulations and Procedure 137, HCPs Platforms may be password-protected, and the HCP may be required to complete a registration process in order to access and use such HCP Platform, all in accordance and as legally mandated.

To the extent this platform is an HCPs Platform, and you are using it as a licensed HCP, Lilly notes that it is strictly prohibited to enable access to the platform, or the information regarding prescription drugs which may be contained in it, to anyone who is not a licensed HCP and/or to transfer to any other person the username and the password enabling access to such information. You are responsible for maintaining the confidentiality of your account and your password, and for restricting access to your computer or the device you use to access and use the platform. You accept responsibility for all activities that occur under your account or password and such use shall be deemed to be use by you.

Patients Websites

Patients Websites are intended for improving the patient's use and adherence to the medicinal treatment prescribed for him/her that includes, inter alia, treatment with a Lilly prescription drug ("Prescription Drugs").

To the extent this platform is a Patients Website, and you are visiting it as a patient prescribed with a Prescription Drug, you must be an adult and legally competent to engage in a binding contract.

Given Regulation 28 to the Pharmacists Regulations and Procedure 137, this platform is password-protected, and only adult patients, which are legally competent to engage in a binding contract, are permitted to use it, after completing an identification process that requires using the password accorded to the patient (such identification process is necessary to ensure that the patient is, in fact, a patient prescribed with a Prescription Drug).

This site may include medical information such as information in relation to the disease for which the patient was prescribed with the Prescription Drug, the Prescription Drug intended for treating such disease, information regarding the appropriate consumption and/or use of the Prescription Drugs, etc. (together: "Medical Information"). The Medical Information is published on this platform in accordance with all applicable laws and regulations, including (without limitation) the Pharmacists Regulations and Procedures 134 and 137. Such Medical Information should not be interpreted and/or attributed with any other purpose.
